Syndesmotic (high) Ankle Sprain
What is it?
The term “high ankle sprain” is often used in athletic settings, but is not well understood by many athletes, coaches, and parents. A high ankle sprain is a syndesmotic sprain. It is an injury to the distal interosseous membrane of the lower leg. Sprain of the inferior tibiofibular ligament often occurs as well. This injury causes instability and widening of the ankle mortise (the space between the tibia and fibula).
Sign and symptoms
• Pain, swelling, and bruising in the front of the ankle
• Pain with walking or standing on the injured leg
Treatment
• Rest, Ice, and Elevation
• Compression with an elastic wrap
• Evaluation by a sports medicine physician – x-rays must be taken of this injury for proper treatment
• Splinting (boot) or casting
• Rehabilitation with a physical therapist or athletic trainer
• Occasionally, surgery to stabilize the lower leg
